It is a T-MOBILE LG G3 smartphone and it has a protective black phone case with a flap that opens / closes over the screen part of the phone. The last time and place that I had my phone was on Monday July 20, 2015 during the afternoon time between 3PM and 5PM at the Walmart located at 5198 Boulder Highway Las Vegas, NV 89122, which is the Walmart that is at the crossroads of Nellis and Boulder Highway.
